Patient Demographics: Male, 60 years old.
Symptoms: The patient presented with high fever, bone pain, and weakness.
Microscopy Findings: 81% blasts, 2% segmented neutrophils, 15% lymphocytes, 2%monocytes.
Blast Morphology: Abnormal promyelocytes are large cells with abundant cytoplasm filled with coarse azurophilic granules and often contain multiple Auer rods that may form so-called faggot cells.
Other Tests: Abnormal DIC profile. Flow cytometry findings are suggestive of Acute Myeloid Leukemia (promyelocytic type).
Diagnosis: The morphological features and flow cytometry results are suggestive of Acute Promyelocytic Leukemia.
How Yumizen H500 Assisted: On the DIFF scattergram of the Yumizen H500 analyzer, the capability to display an extended WBC differential matrix helps identify an isolated population in continuation and emerging from the neutrophil matrix and moving towards the IMG and IMM matrices. The left shift in WBCs, suggested by “Abnormal NEU”, LIC, IMM, and Blast alarms, prompts investigation for a high number of immature WBCs such as blasts. This population corresponds to immature myeloid cells showing high LIC values along with high IMM values. The population discussed shows abnormal promyelocyte morphological features, as observed on microscopy.
