Chase the Case #49 - Hairy Cell Leukemia

New Yumizen H2500 Hematology Analyzer - HORIBA Medical

Yumizen H2500 Hematology Analyzer

October 2025 (PDF for print)

Patient Demography: Female,66 years old

Symptoms: Complaints of left-sided abdominal fullness and pain, loss of appetite, and generalized weakness.

Diagnosis: Lymphoproliferative disorder suggestive of Hairy Cell Leukemia.

Microscopic Review: Peripheral smear revealed atypical lymphoid cells (58%). These cells were intermediate in size, with abundant pale blue cytoplasm and a characteristic hairy or frayed border—fine, irregular cytoplasmic projections. The nuclei appeared round, oval, or indented (reniform/kidney-shaped), displaying the typical "fried-egg" appearance.

How Yumizen Can Assist: The LYM/MON alarm, along with elevated ALY and lymphocytosis, triggered a warning for abnormal WBC differential. The presence of a “lymphocyte peak” on the WBC histogram prompted further microscopic review for abnormal morphology. This well-defined diagnostic pathway facilitated accurate identification of the disorder and supported the correct diagnosis.

This clinical case has been provided by Hematology  Diagnostic Center, Mumbai, India.
