Patient Demographics: Female, 22 years old
Clinical Presentation: A patient with known allergic asthma presented with shortness of breath, wheezing, chest tightness, and a persistent cough.
Microscopy Review: Neutrophils 50%, lymphocytes 15%, monocytes 4%, eosinophils 27%, basophils 1%, metamyelocytes 2%, myelocytes 1%. The absolute eosinophil count of 6.7 × 10³/µL indicates marked eosinophilia.
Diagnosis: Severe allergic asthma
Yumizen Clinical Insights: The LMNE matrix of the Yumizen H500, which provides an extended WBC differential that includes immature cell populations, shows marked eosinophilia. The clear eosinophil cluster is expanded, and some events extend into the large immature cell region. The analyzer generated flags for eosinophilia and large immature cells (LIC and IMG), which prompted a peripheral smear review. The smear review confirmed eosinophilia, mostly mature eosinophils with a few metamyelocytes and myelocytes. This picture is consistent with allergic asthma, and there was no evidence of hematologic malignancy.
Severe allergic asthma with reactive eosinophilic leukocytosis, presenting as marked peripheral eosinophilia with an analyzer-generated large immature cell flag.
